Full lips are considered one of the most important factors in a beautiful smile, while also allowing facial expressions and gestures to come through. With lip augmentation, it is possible to achieve a healthy, attractive, happy and warm expression without significantly altering the characteristic structure of the face. In aesthetic surgery, all operations are performed according to many reference measurements known as the golden ratio. However, since the concept of aesthetics is a subjective concept that can vary according to cultures and individuals, differences also arise. Nevertheless, golden ratio calculations are generally used in aesthetic designs.
The upper and lower lips should have a certain symmetry between them according to the golden ratio. When designing Hera-type lips, the proportions of the philtrum ridge and the eros arch curves must be correct. The eros arch curve located in the centre of the upper lip is made more pronounced. In lips that have been given volume through golden ratio calculations, the lower lip thickness is designed using a 2:1 ratio with the upper lip thickness. The lip contour is made smooth and taut, protruding slightly outward. The upper and lower lip lines are designed to be curved, distinct, and symmetrical.
The amount of teeth and gums visible is one of the most important criteria in lip augmentation. In Hera-type lip augmentation, the upper lip line, i.e. the smile line design, comes to the fore. The ideal upper lip line is a high lip, i.e. a smile line, with less than 3 mm of gum visibility. In lip design, the distance of the lower lip from the teeth is also very important for an ideal smile and lip line. The distance between the lower lip and the teeth should be between 0.5 and 1.5 mm according to ideal measurements. Lips with distinct contours and volume are reshaped with ideal thickness, curve, and angle. The lips have a volume appropriate to the golden ratio from the centre to the edges. This allows for an ideal smile with symmetrical teeth and lip shape.
Since many patients have a deviation in their nose and chin from the centre, determining the correct midline of the lip plays an important role in ideal lip augmentation. The line passing through the Cupid’s bow is the midline of the lip and should intersect with the dental midline for ideal aesthetics. The frontal view should be approximately 2 mm. For symmetry to be achieved when smiling, the corners of the lips must be in harmony with each other. When determining the midline of the lip, tooth shapes also become important. Care is taken to ensure that the midline coincides with the exact centre of the two front teeth.
